UPDATE -- I think it only affected two services in a very specific timeframe, correcting them now. Looks like it went back to being normal afterward so I think it's been fixed.

When I receive a new VPS, is it possible for it to be delivered in an offline state? Leaving a VPS untouched for 16 hours without any setup feels a bit bad to me.

I think it's related to it not passing fraud check and how we have to hook into it to prevent it from doing that, but it doesn't always work out well which would explain the error and lack of explanation by the system. Basically it made it far enough to where it let you pay, then WHMCS does this "smart" sick move of implementing doing fraud check in a terrible way ever since an update a while ago, where it still lets you pay even though it knows it's about to reject it, then it rejects it in the worst way possible, without notifying the user, or the admin, for Stripe.
Let me know if it doesn't provision in like 10-20 minutes after you pay though, I'm not sure how the flash sale system will handle your order if it's in manual review. How it's supposed to work is it doesn't touch it until we mark it off and then it does it afterward.
If the merchant you are paying is bankrupt, your payment would be cancelled automatically.
This happened with Redbox DVD rental service.
For a hosting provider, it means they are deadpooling.

This is another bug. It's not a major bug, it's a bug with price reduction handling. It makes sure it gets the important part done correctly which is actually updating the price on the service that you order, but pays less attention to updating the displayed price. This isn't a big deal as long as prices are dropping, but if prices are going up it can be problematic.
